Questions for off-boarding survey

Hi everyone!

I'm working on creating an off-boarding survey, and I'm curious to know what kind of questions others have put in a similar survey. Right now, we have what I consider standard questions:

• Reason for leaving (multiple choice)
• Rate your experience with the product (1-5)
  • Feedback for responses < 2 (short answer)
